What is Lockout/Tagout (LOTO)?

LOTO is a safety procedure used to ensure machines are properly shut off and can’t be restarted until maintenance or servicing is completed. 

When is LOTO required?

LOTO is required anytime someone is performing servicing or maintenance where unexpected startup or release of stored energy could cause injury. Employee safety is a huge priority. 

Who is authorized to perform LOTO?

Only trained and authorized employees can apply locks and tags. 

What types of energy need to be controlled?

Electrical, mechanical, hydraulic, pneumatic, thermal, chemical, and gravitational energy. 

What is the difference between a lock and a tag?

A lock provides physical restraint, and a tag provides a warning but does not physically prevent operation. 

Do contractors need to follow our LOTO program?

Yes. Host employers must coordinate LOTO responsibilities with contractors. 

What should I do if I see someone bypassing LOTO?

Report it immediately through your company’s safety or ethics reporting system.

No! Only the person who applied the lock can remove it, except under a strict, documented emergency removal process.

Do I need LOTO for minor servicing?

Only if the task meets OSHA’s narrow definition of “minor servicing” and is performed with effective alternative protection. 

What is stored energy, and how do I release it?

Stored energy is residual energy that remains after shutdown. It must be relieved, blocked, bled down, or otherwise neutralized before work begins.

How often do LOTO procedures need to be inspected?

At least annually, according to OSHA requirements. 

What happens if LOTO procedures are not followed?

Injury, equipment damage, OSHA citations, fines, and disciplinary action.
